Icosapent ethyl (Vascepa): how it works, status and trials
ApprovedPillLowering triglycerides
Icosapent ethyl (Vascepa) is an EPA omega-3 taken as 2 g twice daily. It reduces the liver's output of triglyceride-rich particles, and its heart benefit in the REDUCE-IT trial was larger than triglyceride lowering alone explains.
In technical terms: Reduces liver VLDL-TG output; the cardiovascular benefit is larger than TG lowering alone explains.

Where it fits in the body
Triglyceride-rich lipoproteins
VLDL from the liver and chylomicrons from the gut carry triglyceride. Lipoprotein lipase (LPL) on capillary walls breaks it down. ANGPTL3 and apoC-III act as brakes on this process. Removing either brake (11–15) lowers triglycerides, and ANGPTL3 blockade also lowers LDL-C without needing LDL receptors, which matters in homozygous FH.
