Payments, Subscriptions and Refunds

Last updated: October 22, 2026

This page is part of our Terms of Use.

Self-pay only

Preval Health care is self-pay. We do not bill health insurance, Medicare, Medicaid or any other government program, and you agree not to submit our charges to one. You may be able to use an HSA or FSA card; check with your plan administrator. Medicines are paid for at the pharmacy, and lab tests at the laboratory or to us, as shown before you order.

Prices are shown before you pay

Every price, what it includes, and whether it repeats is shown before you confirm a purchase. You can ask for a Good Faith Estimate of expected charges at any time.

Memberships that renew automatically

If you choose a membership or any plan that renews:

  • Before you buy, we show the price, the billing interval, that it renews automatically until you cancel, and how to cancel.
  • You must check a box agreeing to automatic renewal. We email you a confirmation with these terms and the cancellation steps.
  • If a plan has a free trial or introductory price, we tell you the date it ends and the price after, and remind you before the first full-price charge when the law requires it.
  • We send a reminder at least once a year for annual plans, and before any price increase.

How to cancel

  • Online: in your account under Membership, Cancel. It takes effect right away for future renewals.
  • By email: send “Cancel my membership” to support@prevalhealth.com from your account email. We confirm by email.
  • After you cancel, you keep access until the end of the period you already paid for. Cancelling does not end your care relationship; your clinician will help you transition.

Refunds

  • If you don’t meet the eligibility criteria: you are not charged.
  • Before a clinician reviews your intake: full refund of the visit or first membership fee on request.
  • If a clinician decides telehealth care is not right for you at your first visit: full refund of that visit fee.
  • After a completed visit: visit fees are not refundable, because the clinician’s time has been provided, whether or not a prescription is written.
  • Membership renewals: if you forgot to cancel, email us within 7 days of the charge and we will refund it if you have not used the service in the new period.
  • Lab and genetic tests billed by us: refundable until the sample is collected or the kit ships. After that, not refundable.
  • Medicines: paid to and refunded under the policy of the pharmacy that fills them.

Approved refunds go back to the original payment method within 10 business days. State laws that give you more rights still apply.

Preval Plus (iPhone app)

Preval Plus is an optional subscription for extra app features. It is not medical care and does not include visits with a clinician.

  • Prices: $7.99 per month, $49 per year (with a 7-day free trial for new subscribers), or $79 one-time for lifetime access. Prices shown in the App Store at purchase control.
  • Payment is charged to your Apple Account. Monthly and yearly plans renew automatically unless cancelled at least 24 hours before the end of the current period.
  • Cancel anytime in your iPhone Settings, your name, Subscriptions. Refunds for App Store purchases are handled by Apple at reportaproblem.apple.com.
