Notice of Privacy Practices (HIPAA)

Last updated: October 22, 2026

Whose notice applies

Your medical care is provided by independent licensed clinicians who practice through professional entities affiliated with MD Integrations. Those practices are the HIPAA covered entities for your care, and they keep your medical record in their secure electronic health record system. Their Notice of Privacy Practices explains how your health information is used and shared, and your rights.

  • You receive the notice during intake and acknowledge it before your first visit.
  • A copy is available in your patient account at any time.
  • You can request a copy before signing up by emailing privacy@prevalhealth.com.

Preval Health's role

Preval Health (CORDOC LLC) supports the practices with scheduling, billing, customer support and technology. When we handle your health information for them, we do so under a business associate agreement and follow HIPAA and the practices’ notice. We do not sell health information or use it for advertising. Our Privacy Policy covers information outside your medical record, such as waitlist sign-ups.

Your HIPAA rights, in brief

The full notice controls, but you generally have the right to:

  • get a copy of your health record, and ask for corrections;
  • ask to be contacted a certain way, and ask for limits on how information is used or shared;
  • get a list of certain disclosures, and a paper copy of the notice;
  • be notified of a breach of your information;
  • file a complaint with the practice or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints or 1-877-696-6775, without retaliation.
