Terms of Use

Last updated: October 22, 2026

These Terms of Use (“Terms”) are an agreement between you and CORDOC LLC, a North Carolina limited liability company doing business as Preval Health(“Preval Health,” “we,” “us”). They cover prevalhealth.com, the Preval Health iPhone app, and any service that links to these Terms (together, the “Services”).

Please read Section 15. It requires most disputes to be resolved by individual arbitration and waives class actions. You can opt out within 30 days.

If you think you have a medical emergency, call 911 or go to the nearest emergency room. The Services are not for emergencies or urgent care.

1. Who can use the Services

You must be at least 18 years old and able to form a binding contract, and meet the eligibility criteria for clinical services. Clinical services are only available to people physically located in a state where we offer them at the time of each visit (see Who Provides Your Care). By using the Services you agree to these Terms, our Privacy Policy, and, if you become a patient, the Telehealth Informed Consent and the Notice of Privacy Practices.

2. Preval Health does not practice medicine

Preval Health is a health technology company. We run the website and app, write educational content, design care protocols, coordinate scheduling, labs and payments, and provide customer support.

Medical care is provided by independent, licensed clinicians (physicians, nurse practitioners and physician assistants) who practice through professional entities affiliated with MD Integrations(“Treating Clinicians”). Each Treating Clinician uses their own independent professional judgment, including whether telehealth is appropriate for you, whether to order labs, and whether to prescribe any medicine. Preval Health does not control or interfere with that judgment, and no one at Preval Health acts as your treating clinician through the Services.

A clinician-patient relationship begins only when a Treating Clinician accepts you as a patient after reviewing your information. Visiting the website, using the app, taking a quiz or joining the waitlist does not create one.

3. No guarantee of a prescription or outcome

  • A Treating Clinician may decide that you need an in-person exam, more tests, a different treatment, or no treatment. Paying for a visit does not guarantee a prescription.
  • Prescriptions are written only after a clinical evaluation that meets the laws of the state where you are located, and are sent electronically to a licensed U.S. pharmacy.
  • Treating Clinicians do not prescribe controlled substances through the Services.
  • We only offer FDA-approved, commercially manufactured medicines. We do not offer compounded drugs.
  • Results differ from person to person. We do not promise any particular cholesterol level, risk reduction or health result.

4. Your responsibilities as a patient

  • Give complete and accurate information about your health, medicines, allergies and identity, and keep it up to date.
  • Tell your Treating Clinician about side effects or changes in your health, and complete the lab tests your care plan requires.
  • Keep a relationship with a primary care clinician for care outside our scope. The Services cover cholesterol and related heart-risk care only.
  • Use the Services only for yourself. You may not create an account for, or seek treatment on behalf of, another person.

5. Accounts and security

You are responsible for keeping your login details private and for activity under your account. Tell us right away at support@prevalhealth.com if you think someone has used your account without permission. We may verify your identity before a visit or before releasing records.

6. Fees, subscriptions and refunds

Our clinical services are self-pay. We do not bill health insurance, Medicare or Medicaid, and you may not submit our charges to a federal health program. Prices are shown before you pay. Medicine and lab costs are paid to the pharmacy or laboratory unless we say otherwise. Subscription terms, automatic renewal, cancellation and refunds are explained in Payments, Subscriptions and Refunds, which is part of these Terms. You can ask for a Good Faith Estimate of expected charges.

7. Messages and communications

You agree that we and your Treating Clinicians may communicate with you electronically, including by email, in-app message and, if you opt in, text message (see Text Message Terms). Messages are not monitored around the clock. Do not use them for urgent problems.

8. The Preval Health app

The iPhone app is a personal tracking and education tool. Information you enter stays on your device unless you choose to share or export it, as described in the Privacy Policy. The app does not diagnose, treat or give medical advice, and its reminders and charts do not replace your clinician. App purchases are processed by Apple and are also subject to Apple’s Media Services Terms. Apple is not responsible for the app or its support.

9. Educational content

Articles, calculators, guides and videos are general information, reviewed under our Editorial and Advertising Policy. They are not medical advice for your situation. See the Medical Disclaimer.

10. Acceptable use

You agree not to:

  • give false information or impersonate anyone, or try to obtain medicine for someone else or for resale;
  • copy, scrape, reverse engineer or resell the Services or their content, except as the law allows;
  • interfere with the Services, test their security without written permission, or upload malicious code;
  • harass or threaten our staff or clinicians. We may end access for abusive behavior.

11. Intellectual property

We and our licensors own the Services and their content, including text, graphics, logos and software. We give you a limited, personal, non-transferable license to use them for your own non-commercial purposes while you follow these Terms. Feedback you send us may be used without obligation to you.

12. Third-party services

The Services rely on and link to third parties, such as pharmacies, laboratories, payment processors, Apple and the AI provider you choose in the app. Their terms and privacy policies govern their services, and we are not responsible for them.

13. Disclaimers and limitation of liability

EXCEPT AS STATED IN THESE TERMS, THE SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE.” TO THE FULLEST EXTENT THE LAW ALLOWS, WE DISCLAIM IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND NON-INFRINGEMENT, AND WE DO NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED OR ERROR-FREE.

TO THE FULLEST EXTENT THE LAW ALLOWS, PREVAL HEALTH WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL OR PUNITIVE DAMAGES, AND OUR TOTAL LIABILITY FOR ANY CLAIM ARISING FROM THE SERVICES WILL NOT EXCEED THE GREATER OF THE AMOUNT YOU PAID US IN THE 12 MONTHS BEFORE THE CLAIM OR $100. Preval Health is not responsible for the professional judgment of Treating Clinicians, pharmacies or laboratories. Some states do not allow these limits, so they may not apply to you. Nothing in these Terms limits liability that cannot be limited by law.

14. Indemnity

You agree to cover our reasonable losses and legal fees caused by your violation of these Terms or of the law, or by information you knowingly gave falsely, to the extent the law allows.

15. Dispute resolution, arbitration and class action waiver

Talk to us first. Before filing a claim, email legal@prevalhealth.com with a description of the problem. We will try to resolve it within 30 days.

Binding individual arbitration. If we cannot resolve it, you and Preval Health agree that any dispute arising from these Terms or the Services will be resolved by binding arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, before one arbitrator, by video or in the county where you live. We will pay the filing and arbitrator fees beyond what you would pay to file in court. Either party may instead bring an individual claim in small claims court. This section does not apply to claims about the professional medical care provided by a Treating Clinician, which are between you and that clinician and their practice.

Class action waiver. Claims may be brought only individually, not as a plaintiff or class member in any class, collective or representative proceeding, and the arbitrator may not combine claims of different people.

Opt out. You can reject this arbitration agreement by emailing legal@prevalhealth.comwithin 30 days after you first accept these Terms, with your name and the words “arbitration opt out.”

If any part of this section is found unenforceable for a claim, that claim will proceed in court, and the class action waiver still applies to the extent allowed.

16. Governing law and venue

These Terms are governed by the laws of the State of North Carolina and applicable federal law, without regard to conflict-of-law rules. Any court case allowed under these Terms will be brought in the state or federal courts in Wake County, North Carolina, unless the law of your state requires otherwise. State laws that protect you as a patient or consumer still apply.

17. Ending use

You may stop using the Services at any time. We may suspend or end access if you break these Terms or if continuing would be unsafe or unlawful. If your care ends, your Treating Clinician will help with a reasonable transition, and you can request your records. Sections that by their nature should survive will survive.

18. Changes to these Terms

We may update these Terms. We will post the new version with a new date, and for material changes we will notify registered users by email or in the app before they take effect. Changes do not apply to disputes already pending.

19. General terms

These Terms, with the documents they reference, are the entire agreement between you and us about the Services. If a provision is unenforceable, the rest stays in effect. Our failure to enforce a provision is not a waiver. You may not transfer these Terms; we may transfer them in a merger, acquisition or sale of assets.
