Marek Health, and the alternatives

What Marek Health offers and costs as of October 2026, how it handles cholesterol and heart health, and who should pick which.

Online, nationwide in the US. Protocols written by a board-certified preventive cardiologist.

TL;DR

Marek Health runs a coach-guided optimization program that covers hormones, weight, performance and heart health, with a wide menu of medicines that includes rosuvastatin, ezetimibe, bempedoic acid and Repatha. It suits people who want one program for many goals. If lowering your LDL cholesterol (LDL-C) and ApoB is the goal that matters most, a practice that does only lipid care builds everything around that one number.

What Marek Health offers (as of October 2026)

  • Guided Optimization. A clinically supervised program that Marek describes as including physician-prescribed TRT, GLP-1 medicines and therapeutic peptides. A Marek health coach builds a custom lab panel, checks in monthly and coordinates refills and follow-up labs.
  • Heart Health program. After testing and a consultation with a licensed medical provider, clients may be eligible for rosuvastatin, ezetimibe, bempedoic acid, Repatha, fenofibrate, omega-3 ethyl esters, telmisartan, nebivolol, propranolol, canagliflozin and GLP-1 therapy, per Marek's heart-health page.
  • Cost. Marek's heart-health page shows $299 to start treatment online and requires a minimum $450 lab panel purchase. Medicines cost extra. Marek doesn't bill insurance; its FAQ says many clients pay with an FSA or HSA.
  • Requirements. Marek asks every client for a copy of an annual physical from an MD, DO, PA or NP before starting and continuing treatment. Guided Optimization runs in all 50 states, with some treatments limited by state.
  • Self-service labs. You can also order lab panels yourself and review the results without a treatment plan.

Why we start with cholesterol

Heart disease kills more Americans than any other cause, year after year, according to the CDC. Most of it starts with cholesterol-carrying particles (the ones tagged by ApoB, a protein on LDL and its cousins) lodging in artery walls.

Genetic, observational and randomized trial evidence all point the same way: these particles cause atherosclerosis, and the longer and higher your exposure, the more plaque builds (European Atherosclerosis Society consensus, Ference and colleagues, 2017). In the Cholesterol Treatment Trialists' analysis of statin trials, each 39 mg/dL drop in LDL-C cut major heart attacks and strokes by about a fifth. The FOURIER (2017) and ODYSSEY OUTCOMES (2018) trials showed that PCSK9 inhibitors lower events further on top of a statin.

Few longevity strategies have that much evidence behind them, so we put all of our attention there, with an LDL and ApoB goal, the right medicine to reach it, and rechecks that keep you there.

Preval Health may suit you if

Marek offers a wide range of cholesterol medicines, and that serves plenty of people well. We focus every visit on one question: is your ApoB low enough yet, and if not, what changes next?

We focus only on lipid care. A board-certified preventive cardiologist wrote our treatment protocols, and licensed clinicians in our partner telehealth network deliver care and write prescriptions. We order ApoB and Lp(a) at the start, set your goal, and step treatment up from statins through PCSK9 inhibitors until you reach it. We run as a cash practice: $99 a month for care, medicines at cash prices, and no insurance billing.
